Please use this form to register a young person with Inspire and provide consent for them to attend youth work and youth sport sessions

I understand that Inspire may allow other people and organisations to use information that it holds about me: (a) if asked to provide it for legal or regulatory purpose: (b) as part of current or future legal proceedings. I agree that the information I have provided here and throughout the programme can be used by Inspire and other organisations working with them to help run, fund, and evaluate programmes. I understand that data provided here may be combined with data collected through other Inspire programmes and services. I understand I have the right to see any information held about me by Inspire under the General Data Protection Regulations (GDPR) and am aware that Inspire’s full privacy statement can be viewed on Inspire's website.

Inspire’s youth work and youth sport activities provide young people with safe spaces to meet friends, develop skills and have fun. Inspire staff and volunteers will take all reasonable care of the young person whilst they are attending the session but cannot be held responsible for any loss or damage to personal items as a result of any activity.

The Code of Conduct for the sessions attended has been read, and the young person agrees to follow this.

I consent to the young person named above attending and participating in Inspire's youth work and youth sport activities.

I agree to update Inspire of any changes to the above information.

Young people 16 & over can sign this themselves, but emergency contact information must still be received.
Inspire’s youth work and youth sport sessions aim to support young people to grow in skills, confidence and independence. As part of this, sessions are set up to allow young people to arrive and leave independently during and at the end of sessions. [If this is not suitable for the above young person, please ensure that they are informed that they need to stay in the building / at the venue and ensure arrangements are in place for them to be collected].

Providing your consent will grant Inspire unlimited access to photos / videos taken during sessions, quotes, and case studies, unless you ask us to stop using them. A short version of your story / case study may be used on Inspire’s social media platforms; we will change your name if you request us to do so. You can ask Inspire to stop using your case study / quotes / photos / videos at any time, by emailing dataprotection@inspirecharityuk.org or by filing in this form again. If you do, your media will not be used in future, but it may continue to appear in publications already in circulation or shared online. While Inspire makes every effort to ensure all media coverage is positive, we do not have final control over how a journalist or third party may portray you. Examples of how your photos, quotes, videos or written case study may be used:
• Printed marketing materials (e.g. leaflets, brochures, posters).
• On Inspire’s website, social media, or other online sites.
• In the media (e.g. promoting Inspire in the newspaper, TV or radio).
• Printed or online advertising.
• In bids and grant applications for charity programmes.
• By other organisations that want to promote Inspire’s work (e.g. schools, colleges, funding bodies and private sector organisations).
• Where the programme has received funding by a third-party organisation and that organisation has requested media as part of Inspire's reporting requirements.
