Season 2023/24 - Under 18s

 We use this form to make sure all participants can safely and happily enjoy our projects. Participants aged under 18 won’t be able to take part in our projects without a completed form.

Your child has been selected to take part in the above tournament. This tournament will be taking place between 09:00am and 1:30pm on Wednesday 20th March 2024 at Littleon Road Training Ground, Ayrshire Road M7 3RX.

On this day, the participants invited will be picked up from Manchester Enterprise Academy. At 08:20am they will be transported to the venue via Minibus accompanied by Daniyal from Manchester United Foundation. The participants will return via Minibus to Manchester Enterprise Academy to Simonsway, Wythenhawe, Manchester M22 9RH at approximately 01:00pm.

Media

Sometimes we, or Manchester United Football Club (“MUFC”), visit projects to take photographs, film video and take voice recordings of participants. Third parties may also be invited to attend to do the same. These may include our media agencies and service providers, our Commercial Partners, like DXC or adidas, and external media producers such as broadcasters, influencers, content creators and journalists (“Third Parties”). When we do this, we own all rights to the content. We will use the content on our channels and may share with MUFC and the Third Parties. The Third Parties and MUFC will also use the content that they capture or received from us, across their channels and this may be for commercial as well as charitable purposes.

Safeguarding

We believe that it is unacceptable for a child, or adult at risk to experience any kind of abuse. We are responsible for the safety and welfare of your child when they visit our projects. We won’t publish information about a child who is subject to family, care or legal proceedings. We use the information you have written on this form to safely run our projects. We also use personal details to take registers and improve the quality of our projects. We often gather statistics to show how our work makes a difference to children. We are also required to share personal information and statistics with our funding partners. Government Requirements

The Covid 19 pandemic has taught us that situations can change quickly, and that the Government may require data to be provided to manage such situations, for example with the NHS Test and Trace system. Should the Government require the transfer of personal data from Manchester United Foundation, we will only share this in accordance with Government requirements, our legal obligations and our privacy policy.

Media Release 

In consideration of MUF featuring your image in the Material it is agreed as follows:

1.         I consent to MUF (and any of the Manchester United group of companies (“MUFC”)) and the Third Parties; (i) reproducing and broadcasting (by any existing or future media, platform, formats including all forms of digital media, which shall include the Internet or any other online format (“Media”)) the material collected under this Consent Form (“Material”), (ii) to use all the Material or part of it  for commercial and non-commercial purposes and however MUF, MUFC and/or the Third Parties may decide (iii) to use the Material in the fullest way throughout the world in all Media forever.

2.         MUF, MUFC and the Third Parties do not have to use all or any of the Material.

3.         All intellectual property rights and all other rights, title, and interest in the Material shall belong to and be permanently assigned to MUF (including present assignment of future copyright). I agree that I will not make any claim against MUF, or MUFC, or the Third Parties or their assigns for any remuneration in respect of the use of such rights.

4.         I agree that MUF, MUFC and/or the Third Parties can edit, copy, add to, take from, adapt and in any other way alter the Material as they see fit.

5.         I agree to make no comment on social media (e.g. Facebook, Instagram, Tik Tok or Twitter) or to any third party (including the media) about the Material or how the Material is used without MUF’s first giving me permission in writing to do so.

6.         I do not object to MUF, MUFC and Third Parties storing copies of the Material, or in the case of MUF, the contact details provided by this form of consent on its database and using them in the event that MUF needs to contact me in relation to the Material.

7.         In respect of the Material, I waive the benefits of and agree not to assert any so called "Moral Rights" in the Material and such rights under section 77 and section 80 of the Copyright, Designs and Patents Act 1988 (and/or any and all equivalent legislation and/or regulations that may from time to time be in force in any jurisdiction) as I now have or shall in the future acquire in relation to the Material.

8.         I agree that MUF and MUFC shall not be liable to me for any: (i) loss or damage to property; or (ii) death or personal injury; which in either case arises in connection with me providing the Material unless the same is caused by the fault or negligence of MUF or MUFC.

9.         I confirm that I understand the terms of this consent form.

10.      I agree that the benefit of this form of consent may be freely assigned or licensed by MUF to any of the Manchester United group of companies and/or their respective commercial partners without my prior consent.

11.      This form of consent shall be governed by and construed in accordance with English law and the parties submit to the exclusive jurisdiction of the Courts of England and Wales.
