Season 2026/27 - Under 18s

We use this form to make sure all participants can safely and happily enjoy our projects. Participants aged under 18 won’t be able to take part in our projects without a completed form.

We are running this after school club starting in September 2026 at Ulster University Duncreggan Student Village . Each week, this after school club will start at 4pm and will finish at approximately 5pm.

Consent

Please say if you consent for the participant to attend Manchester United Foundation's Girls Football project. If you do consent, you are committing to:

1) Making sure the participant can travel safely to and from the venue.
2) Managing the participant's medical needs before, during and after site visits (Foundation staff will NOT administer medication to children)
3) Letting us know should any of your information change (you can email any changes to consent@mufoundation.org)
4) Agreeing that the participant can take part in all Girls Football projects, both online and in-person
5) Making sure that you have read and agreed to the legal terms at the bottom of this form

When we do this, we own all rights to the content. We will use the content on our channels and may share with MUFC and the Third Parties. The Third Parties and MUFC will also use the content that they capture or received from us, across their channels and this may be for commercial as well as charitable purposes.

Safeguarding

We believe that it is unacceptable for a child, or adult at risk to experience any kind of abuse. We are responsible for the safety and welfare of your child when they visit our projects. We won’t publish information about a child who is subject to family, care or legal proceedings. Data protection notice

We use the information you have written on this form to safely run our projects. We also use personal details to take registers and improve the quality of our projects. We often gather statistics to show how our work makes a difference to children. We are also required to share personal information and statistics with our funding partners. Government Requirements

The Covid 19 pandemic has taught us that situations can change quickly, and that the Government may require data to be provided to manage such situations, for example with the NHS Test and Trace system. Should the Government require the transfer of personal data from Manchester United Foundation, we will only share this in accordance with Government requirements, our legal obligations and our privacy policy.

Media Release 

Thank you for agreeing to allow us to use and share material containing your child’s image and voice (“Material”).  Please read the terms below so that you are aware of how this Material will be used and shared.

1.      You grant MUF (and the Manchester United group of companies (“MUFC”) and any MUFC players in attendance at the Event) and the other third parties mentioned in this consent form and our commercial partners  (DXC and adidas) and charitable partners (Sir Bobby Charlton Foundation and Michael Carrick Foundation) (all together the  “Third Parties”) the right to reproduce, broadcast, publish or share the Material (or any part of it) in any format e.g print, online, social media, video, or any other (whether it exists now or in the future) anywhere in the world, for any purpose (commercial or non-commercial), at any time now or in the future.

2.      MUF and the Third Parties aren’t required to use your image or the Material, even though you’ve given us permission.

3.      All intellectual property rights (including copyright) in the Material belong to MUF. You assign those rights to us now and in the future and agree not to request any further payment for our use of the Material.

4.      MUF (and the Third Parties) can edit, crop, combine, adapt or otherwise alter the Material in any way we or they consider appropriate.

5.      You waive any moral rights in the material under sections 77 and 80 of the UK Copyright, Designs and Patents Act 1988 (and equivalent laws elsewhere) and agree not to object to how we use it.

6.      You agree not to post about, comment on, or discuss the Material or our use of it on social media (such as Instagram or Twitter) or with any third party (including the media) without MUF’s written permission.

7.      MUF (and the Third Parties ) may keep a copy of your image and your contact details in our database, to contact you about the Material if needed.

8.      MUF (and the Manchester United group) won’t be responsible for any loss or damage connected with the Material—unless it’s caused by our negligence.

9.      By signing below, you confirm that you’ve read and understood these terms.

10.  MUF may freely transfer or license its rights under this agreement (including the right to use your image and/or the Material) to any Manchester United group company or third party without asking you again.

11.  This agreement is governed by English law, and any disputes will be decided by the Courts of England and Wales.
