We use this form to make sure all participants can safely and happily enjoy our projects. Participants aged under 18 won’t be able to take part in our projects without a completed form.

Consent

Sometimes we, or Manchester United Football Club (“MUFC”), visit projects to take photographs, film video and take voice recordings of participants. Third parties may also be invited to attend to do the same. These may include our media agencies and service providers, our Commercial Partners, like DXC or adidas, and external media producers such as broadcasters, influencers, content creators and journalists (“Third Parties”). When we do this, we own all rights to the content. We will use the content on our channels and may share with MUFC and the Third Parties. The Third Parties and MUFC will also use the content that they capture or received from us, across their channels and this may be for commercial as well as charitable purposes. Please indicate below if you consent for the participant to take part in media activities by selecting Yes or No, entering your name and providing a signature. If you wish to withdraw consent at any time, please email consent@mufoundation.org, specifying what you are withdrawing consent for.
Please say if you consent for the participant to attend Manchester United Foundation's Street Reds project. If you do consent, you are committing to:
1) Making sure the participant can travel safely to and from Street Reds sites
2) Managing the participant's medical needs before, during and after site visits (Foundation staff will NOT administer medication to children)
3) Letting us know should any of your information change (you can email any changes to consent@mufoundation.org) 4) Agreeing that the participant can take part in all Street Reds projects, both online and in-person
5) Making sure that you have read and agreed to the legal terms at the bottom of this form

Media

Safeguarding

We believe that it is unacceptable for a child, or adult at risk to experience any kind of abuse. We are responsible for the safety and welfare of your child when they visit our projects. We won’t publish information about a child who is subject to family, care or legal proceedings. Data protection notice

We use the information you have written on this form to safely run our projects. We also use personal details to take registers and improve the quality of our projects. We often gather statistics to show how our work makes a difference to children. We are also required to share personal information and statistics with our funding partners. Government Requirements

The Covid 19 pandemic has taught us that situations can change quickly, and that the Government may require data to be provided to manage such situations, for example with the NHS Test and Trace system. Should the Government require the transfer of personal data from Manchester United Foundation, we will only share this in accordance with Government requirements, our legal obligations and our privacy policy.

Media Release 

In consideration of MUF featuring your image in the Material it is agreed as follows:

1. I hereby expressly consent to MUF (and any of the Manchester United group of companies) reproducing and broadcasting (by any media or platform) the Material. You also hereby grant all consents necessary to enable MUF and/or its commercial partners, including DXC and adidas, and its charitable partners including the Michael Carrick Foundation and the Sir Bobby Charlton Foundation,  to use the Material in whole or in part, for commercial and non-commercial purposes and in such manner as MUF and/or its commercial partners may in their absolute discretion decide so as to permit the fullest use throughout the world of the Material (or any part(s) thereof) by all means and in all media and formats (including all forms of digital media, which shall include the Internet or any other online format), whether now known or hereafter developed or discovered, throughout the world for the full period of copyright and all renewals and extensions thereof and thereafter so far as may be possible in perpetuity.
2. I acknowledge that MUF does not have to use all or any of the Material.
3. I hereby agree that all intellectual property rights (including copyright (if any)) and all other rights, title, and interest in respect of the Material shall vest in and are hereby irrevocably assigned to MUF (and this assignment shall operate to the extent necessary as a present assignment of future copyright). I agree that I will not make any claim against MUF (or any of the Manchester United group of companies or DXC Group) or its assigns for any further remuneration in respect of the exploitation of such rights.
4. I agree that MUF shall be entitled to edit, copy, add to, take from, adapt and in any other way alter the Material as they see fit.
5. In respect of the Material, I hereby waive the benefits of and agree not to assert any so called "Moral Rights" in the Material and such rights under section 77 and section 80 of the Copyright, Designs and Patents Act 1988 (and/or any and all equivalent legislation and/or regulations that may from time to time be in force in any jurisdiction) as I now have or shall hereafter acquire in relation to the Material.
6. I agree to make no comment on social media (e.g. Facebook or Twitter) or to any third party (including the media) about the Material or the manner in which the Material is used without MUF’s prior written consent.
7. I do not object to MUF and MUFC storing copies of the Material, or in the case of MUF, the contact details provided by this form of consent on its database in the event that MUF needs to contact me in relation to the Material.
8. I agree that MUF (and/or any of the Manchester United group of companies) shall not be liable to me for any: (i) loss or damage to property; or (ii) death or personal injury; which in either case arises in connection with me providing the Material save to the extent that the same is caused by the fault or negligence of MUF (and/or any of the Manchester United group of companies).
9. I hereby confirm that I understand the terms of this release form.
10. I hereby expressly agree that the benefit of this form of consent (including the right to use the Material in accordance with paragraph 1) may be freely assigned or licensed by MUF to any of the Manchester United group of companies and/or their respective commercial partners without my prior consent.
11. This form of consent shall be governed by and construed in accordance with English law and the parties submit to the exclusive jurisdiction of the Courts of England and Wales.
