To attend one of Achieving for Children’s youth clubs or projects, we need you to complete this registration form. The personal information you provide as part of the registration process will help us to develop targeted services and ensure we are meeting the needs of all young people in our local community. We require information about any disability or medical condition in order to support you or your child to participate safely and enjoy our activities and emergency contact details if anything happens to you when you are with us.
The information will be stored securely and in line with UK data protection law. Further details about how we use your personal information and your rights are available in our privacy notices on our website and displayed at the youth centres. Please ask the youth worker if you would like a hard copy of the privacy notice.
If you are under 13, we need you to take this form home and get your parent or guardian to sign it saying we can hold your information. If you are 13 or over you can give us that consent.
