To participate in an Endeavour programme, each person must have a completed and signed Consent and Participation Information Form. This form helps us cater to individual needs and include everyone.
A participant is anyone attending an Endeavour session(s), which may include:
- A child in an Endeavour programme
- An adult parent / carer of a child in an Endeavour programme
- An adult in an Endeavour programme and /or event
What Consent Means
By signing this form, you agree:
- That you/your child can participate in an Endeavour session(s) and all related activities.
- That you/your child agree to travel by transport provided by Endeavour.
- That you/your child do not have medical or physical conditions that prevent participation.
- That you/your child will follow all safety guidelines from staff.
- That you/your child may receive first aid or medical treatment if necessary.
- That you understand that all activities have a small degree of risk.
- That you have provided accurate information and will update us on any changes.
- That if needed, Endeavour can arrange support from outside agencies or professionals for you/your child.
- That you understand that Endeavour is not responsible for any valuables that you/your child bring and lose, damage, or have stolen during the programme.
- To Endeavour storing, processing and retaining the personal data provided in this form, in line with our privacy policy which is available on our website.
Expectations for Participants
- Be responsible for your/their own behaviour.
- Follow all instructions regarding behaviour, health, safety, and respect.
- Stay with the programme and not leave without informing staff.
- Be open and report any accidents, illness, or discomfort to staff immediately.
We recommend keeping your/your child’s tetanus protection up to date.
